A tragic loss today—Dr. Soha Fatima, a young doctor, left her home as usual, unaware that she would never return. A fire engine collision took her life on the spot near Upparpally, Attapur. She was full of dreams, aspirations, and a commitment to serve, but fate had other plans. Her family must have been immensely proud of her journey—an accomplished doctor, a source of joy and hope. But in a cruel instant, they lost the daughter they cherished. She was taken for postmortem in an auto trolley, and a call from the police would have shattered their world. Answer these: ✅ Do you Snore during Sleep?✅ Do you feel Tired during the day?✅ Do you experience Apneic spells during sleep? Kidney disease is silent but deadly, and by the time symptoms appear, significant damage may have already occurred. As a Diabetologist, I often see diabetes as one of the biggest risk factors for kidney disease. However, early detection, proper management, and lifestyle changes can help prevent or slow its progression. Kidney health is in our hands—let’s act before it’s too late. Diabetes and Kidney Disease: The Hidden Connection Diabetes is the leading cause of Chronic Kidney Disease (CKD) worldwide. Uncontrolled blood sugar damages the small blood vessels in the kidneys, reducing their ability to filter waste effectively. How to Screen for Kidney Disease in Diabetes? Every diabetic patient should have annual kidney function tests, which include:✔ Creatinine & GFR (to assess kidney function)✔ Urine Albumin-to-Creatinine Ratio (UACR) (to check for early kidney damage) Does Good Sugar Control Prevent Kidney Disease? Absolutely! Perfect Time in Range (TIR) and no glycemic variability are better indicators of diabetes control than just HbA1c, which can sometimes be falsely normal. When blood sugars are well-managed, the risk of kidney complications significantly reduces. The Role of Medications in Preventing Kidney Disease Progression If albuminuria (protein in urine) is detected, early intervention is crucial to prevent CKD progression. The good news is that we now have effective drugs that can slow or even halt kidney damage, such as:✅ SGLT2 inhibitors (Dapagliflozin, Empagliflozin) – Kidney-protective, reduce progression of CKD✅ Finerenone (Kerendia) – A newer non-steroidal mineralocorticoid receptor antagonist that reduces CKD progression in diabetic patients✅ RAAS blockers (ACE inhibitors, ARBs) – Help lower BP and reduce protein loss in urine Once the Kidney is Damaged, Can It Be Reversed? No, CKD is irreversible, which is why prevention is key. Timely screening, lifestyle modifications, and appropriate medications can help maintain kidney function for as long as possible. The ABCs of Kidney Health: A1c, Blood Pressure, and Cholesterol To prevent or manage CKD, we must take care of the ABC of kidney health:✔ A1c (Blood Sugar Control): Keep diabetes under control to prevent kidney damage.✔ Blood Pressure: High BP can worsen kidney disease; target <130/80 mmHg.✔ Cholesterol: High cholesterol contributes to kidney and heart disease; statins may be needed. Other Major Causes of Kidney Disease While diabetes and hypertension are the leading causes of kidney disease, several other conditions and risk factors can also lead to Chronic Kidney Disease (CKD) or Acute Kidney Injury (AKI). 1. Painkillers & Medications (Drug-Induced Kidney Damage) Non-Steroidal Anti-Inflammatory Drugs (NSAIDs) like Ibuprofen, Diclofenac, Naproxen – Can reduce blood flow to the kidneys and worsen CKD. Antibiotics (Aminoglycosides, Vancomycin) – Can cause acute kidney injury (AKI), especially in elderly or dehydrated patients. Certain Chemotherapy & Immunosuppressive Drugs – Can lead to nephrotoxicity and kidney damage. 🔹 Who should avoid NSAIDs? People with existing kidney disease (CKD, diabetic nephropathy). Elderly patients, as kidney function declines with age. 🔹 Who can take NSAIDs safely? People with normal kidney function can take NSAIDs for short durations without risk. 2. Infections Affecting the Kidneys Recurrent Urinary Tract Infections (UTIs) – Can cause pyelonephritis (kidney infection), leading to scarring and permanent kidney damage. Post-Streptococcal Glomerulonephritis (PSGN) – A kidney inflammation after strep throat or skin infections. 🔹 Prevention: Treat UTIs promptly to prevent kidney involvement. Good hygiene and infection control practices. 3. Genetic & Autoimmune Kidney Diseases Polycystic Kidney Disease (PKD) – A genetic disorder where cysts form in the kidneys, gradually impairing function. Lupus Nephritis (SLE-related CKD) – Autoimmune attack on the kidneys, common in young women with lupus. IgA Nephropathy (Berger’s Disease) – A slow-progressing kidney disease due to immune system dysfunction. 🔹 Management: Genetic testing and family screening for PKD. Early diagnosis of autoimmune conditions like lupus. Regular kidney monitoring in autoimmune diseases. 4. Kidney Stones & Obstruction Kidney stones (Nephrolithiasis) – Large stones can block urine flow, leading to kidney damage over time. Prostate Enlargement (BPH) – Can obstruct urine flow, causing kidney damage if untreated. Urinary Tract Obstruction (Strictures, Tumors) – Blocked urine can back up into the kidneys, leading to hydronephrosis. 🔹 Prevention: Drink enough water to prevent stone formation. Early treatment of prostate issues in older men. Timely surgery for urinary obstructions if needed. 5. Lifestyle Factors & Toxins That Harm Kidneys Dehydration & Low Water Intake – Can lead to kidney stones and reduced kidney function. Excessive Alcohol & Smoking – Both increase the risk of kidney disease and kidney cancer. Heavy Metal Exposure (Lead, Mercury, Cadmium) – Common in industrial workers, leads to chronic kidney damage. Herbal Supplements & Ayurvedic Medicines – Some contain nephrotoxic heavy metals, leading to CKD. 🔹 How to protect your kidneys? ✅ Drink 2-3 liters of water daily (unless advised otherwise).✅ Avoid excessive alcohol and smoking.✅ Be cautious with unregulated herbal & alternative medicines.✅ Reduce exposure to toxic metals & pollutants. A little care today can prevent serious problems tomorrow — listen to your body and give your kidneys the attention they deserve.” What Happens When Kidney Disease Progresses to Kidney Failure? If kidney function drops below 15% (End-Stage Renal Disease – ESRD), the only options are: ✔ Kidney Transplantation – The best long-term solution✔ Renal Replacement Therapy (Dialysis) – Needed until a transplant is possible Delaying dialysis can be dangerous! Despite advancements in healthcare, women’s health issues—especially diabetes, heart disease, obesity, and metabolic disorders—often remain underdiagnosed, undertreated, and overlooked. As a Physician and Diabetologist, I see firsthand how gender disparities affect women’s health outcomes. 1. The Silent Epidemic: Women and Diabetes Diabetes is different in women: Women with diabetes have a higher risk of heart disease, stroke, and kidney disease than men. Gestational diabetes (GDM) is on the rise and increases the risk of type 2 diabetes later in life. PCOS & Insulin Resistance: A major but neglected issue leading to early diabetes, obesity, and infertility. Why are women with diabetes at greater risk? Hormonal influences affect glucose metabolism. Societal & cultural barriers lead to late diagnosis and poor follow-up. Weight loss struggles are common due to metabolic differences. Call to action: Women must get regular screenings (HbA1c, lipid profile, BP checks) and be more proactive about their health. 2. Women and Heart Disease: The Hidden Killer Heart failure and heart attacks are deadlier in women than in men. Women are less likely to receive guideline-directed therapy for heart disease. Symptoms in women are often atypical (nausea, fatigue, breathlessness) and misdiagnosed as anxiety or stress. What can be done? Encourage heart health screenings for all women, especially diabetics. Promote awareness of atypical heart symptoms. More emphasis on exercise, diet, and lifestyle modifications. 3. The Obesity Epidemic in Women Obesity rates are increasing, especially in urban women, leading to metabolic disorders. Why is weight loss harder for women? Hormonal changes (PCOS, pregnancy, menopause). Emotional eating and stress-related weight gain. Limited focus on strength training and metabolic conditioning. The way forward: Focus on sustainable lifestyle changes rather than crash diets. Address mental health & stress as key contributors to obesity. Weight loss medications & metabolic therapies should not be stigmatized. 4. Women’s Health: A Family Health Issue Women often prioritize family health over their own. Healthcare decisions are delayed due to responsibilities and financial constraints. A woman’s health affects the whole family—from pregnancy to menopause, every stage requires proactive care. “Empowering women begins with prioritizing their health—not just on Women’s Day, but every day. True celebration is ensuring access, care, and well-being beyond the occasion.” Helping Women Prioritize Their Health: A Collective Responsibility Women are the backbone of families and communities, but their health often takes a backseat due to responsibilities, societal expectations, and lack of awareness. This Women’s Day, I urge everyone—husbands, fathers, brothers, sons, employers, and friends—to: ✅ Encourage women to seek regular checkups instead of waiting for symptoms.✅ Support their health decisions—whether it’s dietary changes, fitness goals, or medical treatments.✅ Create a positive environment for self-care—physical, mental, and emotional.✅ Break the stigma around obesity, diabetes, and heart health in women—early intervention saves lives.
